If you are looking to get involved this year, you've come to the right place.  Here are the forms we need to have filled.  You can print a blank copy and write on them or fill them out on your computer. Either way, you can bring a pre-filled form with you to auditions to speed up your journey through our process. You will need the forms to begin training with us and to participate in any of our shows.

Waiver

This form will need to be filled out at or before auditions. Bringing a photo ID will help expedite the process, and those under 18 need to be accompanied by a parent or guardian with a photo ID who will also fill out parts of this form.​

​This form is relevant to the audition process as it asks for your preferences as well as you credentials. Take your time — it will make your audition easier.

Medical Form

This form asks for some basic medical and insurance information. Much like sports, dance and/or martial arts, stage and stunt combat can be physically demanding.  Although a sports physical is not required to participate, it is always good to check with your doctor before starting any new program.​
